The PropertySheet is not fully painted with the selected theme, even in the sample application there is a grey area on top beneath the page buttons. Is there a way to paint the background correctly? Thanks.

The CExtResizablePropertySheet and CExtResizablePropertyPage classes really have appearance compatible with 2000 and native XP paint managers only if the property sheet uses the tab control-based mode. They are theme compatible in the wizard mode only. So, you can use the dialog window with the CExtTabPageContainerWnd and CExtButton windows inside. All these windows should be anchored and page windows should be created as children of the tab page container.
