Hello. I describe a reproduction method in detail. (1), You move the following menus. 
(2), You add an accelerator key. 
(3), You choose the following menus. ( MenuRebuild works. ) 
(4), You move the following menus. 
(5), You choose a Quick accelerator key menu.
