The multi-line tabs featurer is supported in CExtTabWnd only partially (not yet completely finished). You can download this project which demonstrates this feature:

MultilineTabTest

Please note, the Microsoft’s UI guidelines for Windows Vista do not recommend to use multiline tabs at all, do not use scrollable single line tabs and do not use tabs at all if there are more than 8 tab items in the tab window. The vertical list box-like window for switching some content according to the currently selected list box item is called vertical tabs in terms of Windows Vista. This style of tabs assumed to be more convenient and user friendly. Such tabs are used in the ribbon options dialog which you can see in the RibbonBar sample application if you invoke the File button and click the Options button at the bottom of the displayed menu window.
