Hello.

At the time of printing
I want to set the paper of the printer in A3.
Will CExtPPVW be possible?

Hello.

With GetPrinterDeviceDefaults, I was able to change "Paper Size"
I cannot change "Output Size".

The CExtPPVW_Printable::OnPreparePrinting() method uses CWinApp::GetPrinterDeviceDefaults() to retrieve the printer settings. So you should simply configure the default printer for using A3 format before printing/previewing.
