In my application, ArcGIS toolbar control and Map control are contained in a ATL dialog (derived from CAxDialogImpl) which is a page inserted into a CExtTabPageContainerWhidbeyWnd.
But if it is implemented as modal dialog without inserting into CExtTabPageContainerWhidbeyWnd, they do show up.
